Description for: Direct Service Worker
This position reports
directly to the DSW Director and is a Part-time position.
Qualifications: The most
qualified applicant will have a high school diploma or equivalent document,
and demonstrate experience or a personal knowledge with disabilities. A
certificate of Nursing Assistant Training is preferred. The individual has
provided services to persons with loss of sensory and/or motor functions
interfering with activities of daily living. He or she must be committed to
the belief that persons with disabilities -- including spinal cord injury,
stroke, amputation, cerebral palsy, multiple sclerosis, muscular dystrophy,
and brain injury -- have the ability to direct their own care and their own
lives and live independently in their own homes with proper assistance.
Responsibilities may
include:
Assist consumer with
personal care activities, including but not limited to: bathing, dressing,
grooming, as well as bowel and bladder care.
Assist consumer with
transfers.
Assist consumer with
housekeeping activities, including but not limited to: sweeping, mopping,
dusting and general straightening.
Assist consumer with
laundry activities, including but not limited to: washing, drying, folding,
and putting away laundry, changing linen, mending clothes, and ironing
clothes.
Assist consumer with
meals, including but not limited to: cooking, setting up, feeding, storing
meals, and washing dishes.
Assist consumer with
shopping, including but not limited to: grocery shopping, and personal
shopping.
Assist consumer with
transportation, including but not limited to: driving consumer’s own
vehicle, transporting consumer in DSW’s own vehicle, and/or assisting
consumer with public transportation.
Assist consumer with
recreation, including but not limited to: in home and/or community
activities.
Assist consumer with
paperwork, including but not limited to: paying bills, writing letters,
completing forms.
Turn in timesheet and
service log to consumer or their representative for consumer/representative
to check and sign every two weeks.
Turn in a complete,
correct, and legible original timesheet and service log to the SLIC office
by the timesheet due date.
Attend 40
hours of approved training per year.
